What the tear film actually is
Dry eye disease is defined as a multifactorial disease of the ocular surface characterised by a loss of homeostasis of the tear film, in which tear film instability and hyperosmolarity, ocular surface inflammation and damage, and neurosensory abnormalities play aetiological roles.1 In plainer terms: the tear film is a layered structure, and when any layer stops doing its job the surface of the eye suffers.
That is why "dry eye" is a misleading name. Plenty of people with it have watering eyes. The issue isn't always the amount of tears — it's their quality and stability.
The two common patterns
Evaporative. The oily lipid layer is thin or poor quality, usually because the meibomian glands in the lid margins aren't secreting properly. Tears evaporate too fast. This is the more common pattern, and it's the one screen work makes worse.
Aqueous-deficient. Not enough tear volume is being produced. Associated with age, some medications and some systemic conditions.
Many people have a mix of both, which is one reason a single over-the-counter drop so often disappoints.
What management looks like
The published management framework is staged — you start with the simplest interventions and escalate only if they don't hold.2 In practice that means starting with lid hygiene, warm compresses, blink habits and environmental changes, adding the right lubricant at the right frequency, and reviewing contact lens wear if lenses are part of the picture. Four things that cost nothing
- Lower the monitor so you look slightly down — less eye surface exposed
- Deliberate full blinks, a few times an hour
- Move the air-conditioning vent so it isn't blowing at your face
- Warm compress on the closed lids for a few minutes, daily
If symptoms persist despite these, get the eyes examined rather than escalating drops on your own.

What people ask us.
My eyes water constantly. Can that be dry eye?
Yes, and it's one of the most common surprises. When the ocular surface is irritated, the eye can produce a flood of reflex tears that lack the oily layer needed to stay put. The eye is watering and dry at the same time.
Are over-the-counter drops enough?
They help symptoms but rarely address the cause. Which drop, how often, and whether preservative-free matters all depend on what is actually driving the dryness. Using the wrong one for months is a common reason people conclude 'nothing works'.
Does screen time really cause dry eye?
It contributes. Concentrating on a screen reduces blink rate and increases incomplete blinks, so the tear film isn't being spread or refreshed properly. Digital eye strain and dry eye overlap heavily in people who work at screens.
Can I still wear contact lenses with dry eye?
Often yes, but the lens material, the replacement schedule and the care solution all need reviewing. Sometimes the lenses are contributing to the problem; sometimes a different material solves it entirely.
How long before it improves?
Symptomatic relief can be quick. Changing the underlying gland function takes longer — think weeks of consistent lid care rather than days. That is normal, and it is the part people abandon too early.
